Change Can Happen Inside The Gates
Many individuals and families in our community who are homeless, or at risk of becoming homeless, are struggling. They’re “falling through the cracks” due to the gaps in needed services. Together we can help these vulnerable families by opening the Gates to Change Outreach and closing those gaps!
Helping and empowering those experiencing homelessness or at risk of becoming homeless to make sustainable life changes.
Initial Services
SSI/SSDI Outreach, Access, and Recovery (SOAR) promotes recovery and wellness through increased access to Social Security disability benefits for eligible individuals who are experiencing homelessness OR at risk of becoming homeless AND have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder.
Resource referrals will be provided by request or based on the needs of the GTCO participants
